Business details such as Health Canada Licence number, company name, address and phone number are to be filled in the Chain of Custody Form that is e-mailed to you. It is important to fill the COC form in its entirety for full traceability. Please include strain name or variety, as well as the lot number of the sample.
Under no circumstances will we share your information or sample results to a third-party. Only those listed in the COC form will be contacted. Do not share information with anyone from outside of our organization.
