For quality assurance purposes, it is important to fill all or as many fields in the chain of custody form as possible including your Health Canada issued licence number, contact information and shipping address. Ensure the exact weight of your sample (to one tenth of a gram), name and Lot # are correct for full traceability of sample movement. These fields are populated into the final CoA and must reflect the correct identity of the sample being tested. Missing or erroneous information will affect the final release of your product.
Pack your sample, a copy of your invoice or receipt and corresponding chain of custody form into a mailer and send to our facility. Quoted turn-around times begin upon receipt of sample. To ensure the fastest turn-around, samples begin processing upon receipt. Your CoA is immediately issued to your e-mail when tests are complete. In the case of payment by PO, CoA is issued upon receipt of payment.
